OUT OF NETWORK INSURANCE:
If we are not contracted with your PPO insurance, we encourage you to contact them directly to see if your plan provides out-of-network benefits.
CASH PATIENTS:
If you do not have insurance, or have insurance with which we are not contracted, our office offers reasonable cash rates for new and return patient visits. If a biopsy or procedure is performed this may be at an additional charge. Any charges will be explained prior to performing any procedures.
